At CSA, we always welcome your feedback. We would appreciate a few moments of your time to complete the following surveys about your player’s staff coach and DOC. If you have more than one player in the club, please complete a survey for each coach and DOC (Director of Coaching).

Please be honest with your assessment. It allows us to look at things objectively, listen to our membership and react accordingly. The evaluations are routed to our senior administrative staff who will compile the responses and forward them to the appropriate person. If the evaluation is referencing a Director of Coaching, your responses will go directly to the Executive Director, Brad Wylde. If your responses are referencing a staff coach, the compiled responses will go to the Director of Coaching for that coach. We do not track your IP address or in any other way identify the person submitting the response.

Evaluations will close on Friday, December 12th.
